Job Title: Per Diem Pharmacist

Location: Vital Care Infusion Services of Scranton

Type: Per Diem

Availability: Monday - Friday, 8:00 AM - 5:00 PM

Job Description:

We are seeking a qualified and experienced Per Diem Pharmacist to join our team. The ideal candidate will have a background in home infusion therapy and be available to work Monday through Friday from 8:00 AM to 5:00 PM. As a Per Diem Pharmacist, you will play a crucial role in the preparation, dispensing, and management of infusion medications, ensuring the highest standards of patient care and safety. This position will be on an as needed basis.

Responsibilities:
Prepare and dispense infusion medications in accordance with physician orders and established protocols.
Conduct clinical assessments and provide patient education regarding infusion therapies.
Monitor patient responses to therapy and collaborate with healthcare providers to optimize treatment plans.
Maintain accurate and complete patient records, including medication profiles and clinical notes.
Ensure compliance with all regulatory and accreditation standards related to infusion therapy.
Oversee the sterile compounding process and ensure the integrity and quality of compounded medications.
Manage inventory of medications and supplies, including ordering and stocking as needed.
Provide exceptional customer service and support to patients, caregivers, and healthcare providers.
Participate in on-call as needed to support patient care outside of regular business hours.

Qualifications:
Doctor of Pharmacy (Pharm.D.) or Bachelor of Pharmacy (RPh) degree from an accredited institution.
Current and active pharmacist license in the state of Pennsylvania.
Experience in home infusion therapy or a related field.
Strong knowledge of sterile compounding techniques and aseptic processing.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Detail-oriented with strong organizational and time-management skills.
Proficiency in pharmacy management software and electronic health records (EHR) systems. Experience with CareTend a plus.

Preferred Qualifications:
Experience with patient education and counseling in a home healthcare setting.
Familiarity with infusion pumps and related equipment.

Pay: $50.00 - $65.00 per hour
