1ST LINE SUPERVISOR: DIRECTOR OF NURSING

WORK AREA: ENTIRE FACILITY

PURPOSE:

To identify and oversee the restorative nursing care needs of each resident.

WORKING HOURS:

80 hours per pay period or as assigned by supervisor.

THIS PERSON REPORTS TO:

Director of Nursing

HIPAA MINIMUM NECESSARY:

Responsible to adhere to HIPAA MINIMUM NECESSARY guidelines and safeguard protected health information.

WORKING CONDITIONS:

Well lit, well ventilated office. Will work in residents rooms, dining areas, and through out the facility during assessment and evaluation procedures. May be subject to infectious disease, odors, emotional outbursts, and other personality traits of the aged. Required to follow the No Lift Policy and must be on constant alert to reduce risk of resident injuries. May stand, stoop, bend and walk during the workday.

DUTIES AND RESPONSIBILITIES:

1. Serves as the coordinator for restorative nursing care and services provided to the residents of the facility.

2. Serves as liaison between the licensed therapist(s) (PT, QT, ST) and facility staff.

3. Works closely with the resident Plan of Care Team related to restorative nursing care for accurate coding of appropriate sections of the MDS.

4. Coordinates and supervises all restorative nursing staff and services.

5. Coordinates timeliness of completion of restorative evaluations, initiation of care and services, restorative plans and progress notes.

6. Screens residents for restorative nursing programs utilizing RAI manual guidelines.

7. Documents NJO nursing orders to begin and/or discontinue restorative programs and enter into EMR and updates care guides as indicated.

8. May chair facility restorative QA meetings (usually a part of the facilitys Resident status/Medicare meeting) as assigned.

9. Assists with training oversight and instruction of Restorative Aides, (C)NAs and Licensed Nurses as indicated in carrying out restorative nursing programs and completion of appropriate program documentation.

10. Assures completion of periodic reviews (at least quarterly with MDS schedule), of Restorative nursing programs.

11. Discharges or transfer of residents from restorative nursing programs as appropriate.

12. Communicates changes in Restorative programs to DON, Dietary Manager, RACs and RPOC team as they occur.

13. Conducts ongoing screening/observation of residents in restorative nursing programs.

14. May coordinate and oversee weight program (both weekly and monthly), and communicates to the QI team weekly.

15. May oversee psychotropic medication behavior monitoring documentation and AIMS testing.

16. Screens for functional range of motion status of residents on admission, readmission and at least quarterly.

17. Coordinates and oversees resident needs for devices and/or other interventions, and reports to QI meeting weekly.

18. Demonstrate competency using current clinical software system.

19. May carry out Restorative nursing programs as indicated.

20. Other duties as assigned by supervisor.
