HCW Qualifications:

· Experience: One year of practical nurse experience after graduation. Shall have practiced as an LPN performing hands-on patient care in the outpatient clinical setting for at least 12 months, within the past 24 months. Experience performing care and treatment of patients; operating and maintaining therapeutic equipment; properly administering oral and parenteral medications; conducting paraprofessional portions of physical examinations; or assisting in medical treatment of patients in accordance with established procedures and protocols, within the LPN scope of practice. As appropriate within established procedures, protocols and scope of practice, must have understanding of and ability to correctly utilize and/or apply; medical terminology, anatomy and physiology, psychology, pharmacology, medical ethics and legal concepts, clinical procedures, patient care; emergency medical treatment to include cardiopulmonary resuscitation, aseptic technique, specimen collection, diagnostic testing, prepare and administer oral and parenteral (excluding IV) medications, vaccination procedures, mechanisms of anaphylaxis and shock and their treatment; prescription drugs and their administration; operating and maintaining therapeutic equipment, risk management, medical contingency operations, computer operations in the use of basic word processing, data entry and automated medical records

· Licensure/Registration: Current, full, active, and unrestricted license as a practical nurse. HCW Duties (5.1 Task 1): The duties for the HCW are as follows:

· Initiate, perform, and complete assigned duties in providing care to variable patient populations. Serves as a Practical Nurse performing patient care and nursing administrative duties for outpatients in accordance with established policies, procedures, protocols, and guidelines.

· Establishes and maintains appropriate, professional, interpersonal relationships with co-workers, families, peers, and other team members.

· Participates in training and continuing education programs for new procedures, techniques, and equipment.

· Assists providers with patient referral tracking, coordinates off-base care on an as-needed basis for urgent/emergent referrals. Assists with telephone consults (with exclusion of symptom-based triage) as needed.

· Maintains current knowledge of preventive screening standards and current trends in population-based health management. Provides individualized patient teaching on appropriate topics to include preventative healthcare and health maintenance.

· Reviews scheduled appointments for following week, plans, intervenes, and contacts patient as needed. Reviews patient record prior to appointment, reviews for Periodic Health Assessment, Individual Medical Readiness, deficiencies, preventive health needs, completion of referrals, medication renewals, and needed studies. Identifies and records patient problems and appropriately notifies provider/nursing Team of observations. Participates in team huddles and care coordination team meetings as appropriate.

· Assists with obtaining initial medical history, assesses/records vital signs, and updates patient’s medical record promptly and accurately. Assists with patient check-ins, admissions, and transfers as directed. Prioritizes the urgency of patient needs and facilitates care at the appropriate time and location. Recognizes and responds to emergencies and performs basic life support measures.

· Ensures all equipment is properly assembled, operates, cleaned, and stored at the end of each work shift. Ensures preventive maintenance and routine cleaning are performed as directed.

· Partners with appropriate departments to assess/select appropriate community health services for patient referrals. Conducts follow-up with referred patients to evaluate effectiveness.

· Complete assigned patient care based on the patient’s conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report, and record relevant patient information.

· Observes, identifies, and respond to the patient’s needs for care, including medication, equipment-assisted care, and patient/family education. Records treatment, procedure rendered, and observed effects.

· Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform treatments according to established policies/procedures.

· Recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.

· Perform support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing, and handling specialized instruments or other specialized equipment, and monitoring the patient’s condition before, during, and following the procedure.

· Orient peers and ancillary personnel to unit policies and procedures, skills, and standards of practice.

· Integrate healthy work environment principles that result in improved patient healthcare delivery, education of patients and their families and employee satisfaction.

· Adheres to infection prevention and control procedures including use of personal protective equipment, disposal of waste, initial processing/preparation of reusable medical instruments for sterilization, and aseptic techniques.

· Coordinate Patient Separation History and Physical Exam; to include medical record retrieval and review, scheduling, collaboration with Physical Evaluation Board Liaison Officer and specialty clinics, for members of all military branches, in accordance with DoDI 6040.46.
