Home Health: Start of Care Registered Nurse (Full Time)

Millennium Home Care (an affiliate of Millennium Healthcare, LLC)Start of Care Registered Nurse (RN)Millennium Home Care, a leading provider of compassionate home healthcare services, is seeking a skilled and dedicated Registered Nurse (RN) to join our team as a Start of Care Nurse. In this pivotal role, you will be responsible for initiating home health care services, conducting comprehensive OASIS assessments, and developing individualized care plans for our patients.Responsibilities:* Conduct thorough initial assessments and OASIS evaluations to determine patient needs and eligibility for home health services.* Develop and implement comprehensive care plans based on assessment findings, collaborating with physicians and other healthcare providers.* Provide skilled nursing care, including medication administration, wound care, and monitoring vital signs.* Educate patients and caregivers about disease management, medication administration, and self-care techniques.* Accurately document patient assessments, interventions, and outcomes in the electronic medical record.* Collaborate with the interdisciplinary team to ensure optimal patient care and outcomes.Qualifications:* Active and unrestricted Registered Nurse (RN) license in the state of Florida.* Minimum of 1 year of nursing experience, with a strong preference for home health or acute care.* Demonstrated proficiency in OASIS assessments and documentation.* Excellent clinical assessment, problem-solving, and decision-making skills.* Strong interpersonal and communication skills to build rapport with patients and caregivers.* Ability to work independently and manage a varied caseload.* Proficient in electronic medical record (EMR) systems.* Valid driver's license and reliable transportation.Benefits:* Full medical, dental, and vision benefits* Competitive 401(k) plan with company match* 3 weeks of PTO + 7 holidays per year* Stipends to cover gas, mileage, and phone usage* iPad to complete documentation in the patient's homeAt Millennium Home Care, we are committed to providing our patients with the highest quality of care.
