*Hospice On Call RN*

*Reports To: *Director of Clinical Services

*Shift: On-Call After Hours*

*Location: Yancey, Mitchell, Madison and Northern Buncombe Counties*

*Position Summary:*

This is an On Call position nights & weekend, every other week. 7 days on/7 days off. Assist with providing nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.

*Essential Duties & Responsibilities:*
* Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
* Serve as back-up responder for symptom-based calls from current patients after normal office hours
* Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
* Participate in implementation and coordination of patient’s hospice care
* Assist with emergency admissions
* Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
* Record and report the results of the nursing assessment.
* Plan, initiate, deliver, and evaluate appropriate nursing acts.
* Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
* Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
* Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
* Provide teaching and counseling about the patient's health.
* Report and record the plan for care, nursing care given, and the patient's response to that care.
* Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
* Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.

*Knowledge, Skills & Abilities:*
* Must be willing and able to care for patients with infectious disease processes unless approved accommodations
* Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
* Knowledge of HIPAA privacy rules and regulations
* Knowledge of electronic medical records systems
* Demonstrate basic data entry skills
* Demonstrate excellent organization skills
* Demonstrate excellent interpersonal skills
* Demonstrate excellent written and oral communication skills
* Ability to work under pressure to meet deadlines
* Ability to take initiative and utilize innovative techniques and ingenuity
* Ability to interact with various departments throughout the organization

* Ability to participate as a team player to coordinate projects
* Ability to plan, prioritize and coordinate multiple projects
* Demonstrate skill/expertise in pain and symptom management
* Demonstrate ability to effectively collaborate and facilitate the work of a group
* Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation

*Minimum Qualifications:*

* Valid NC RN license
* Valid Driver’s License and proof of automobile insurance

Job Type: Full-time

Pay: $35.00 - $38.00 per hour

Expected hours: No less than 32 per week
