Job Summary

We are seeking a skilled and empathetic Hospice RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services, empower patient caregivers, and collaborate with healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities
  • Provide direct patient care services, including evaluating patient and family needs, developing and implementing individualized care plans, and managing patient clinical records.
  • Act as a liaison between physicians, patients, and families, ensuring seamless communication and coordination of care.
  • Empower patient caregivers through education and support, helping them navigate the complexities of hospice care.
  • Collaborate with an interdisciplinary team, including LPNs, CNAs, and support staff, to provide comprehensive care and support to patients and families.
  • Participate in quality assessment and performance improvement initiatives, ensuring that our care meets the highest standards of quality and compassion.
Requirements
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Current driver's license and automobile insurance
  • Current CPR certification
