GENERAL DESCRIPTION

Serves in the Behavioral Health Division under the clinical direction of the Medical Services Director and the administrative direction of the Center Behavioral Health Director. Performs advanced (senior-level) nursing work. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. This position is clinically responsible for the provision of nursing services to the Centers Adult Behavioral Health and more specifically the Assertive Community Treatment Team (ACT). The ACT RN will promote program goals to instill hope, improve self-sufficiency, increase ability to live independently in community, and increase periods of abstinence from substances. ACT uses a multidisciplinary approach (e.g., psychiatric, substance use, skills development) in one mobile service delivery team. This mobile unit supports individuals with the most severe and persistent mental illness by providing intensive treatment, rehabilitation and support services at consumers' homes and other community settings. The position also serves as consultant to the Utilization Management Division of the Center as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Gathers and maintains patient information and records. Collaborates with the interdisciplinary team to discuss treatment plans and progress. Analyzes and evaluates patient data or test findings to assist in treatment process. Provides psychiatric nursing services to adult persons with mental illness and dual diagnosis of mental illness with substance abuse and/or intellectual or developmental disability. Completes documentation of services in the client record within Center UM guidelines and timeframes.
Coordinates and reviews nursing activities and makes recommendations for improvements. Serves as an expert consultant, instructor and mentor to Center staff and (with proper consents) to outside persons regarding medical/psychiatric needs of consumers.
Participates in the Centers Utilization Management (UM) processes, especially as they relate to the resiliency and disease management (RDM) model of mental health services
Maintains positive, professional, and effective relationships with fellow members of the Center medical team, clinical and support staff, administration, and with community stakeholders.
Communication expectation to give full attention to what other people say, takes time to understand the point being made, and, asks questions as appropriate.
Serves as a resource on nursing practices and education programs by performing consultative and administrative work.
May assign and/or supervise the work of others.
Must stay compliant with all required training.
Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required for the 8-5 work day or agreed upon hours 100% of the time, to ensure consistency and completeness of programs processes.
Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.
Frequent daily driving within the catchment area is required.

GENERAL QUALIFICATIONS

Experience/Education/Licensing/Certification

Graduation from an accredited four-year college or university with major course work in nursing(BSN) or completion of an accredited ADN nursing program.
Must be registered as a Registered Nurse in the State of Texas or a state that recognized reciprocity through the Nurse Licensure Compact.
Experience in psychiatric nursing work is preferred.
Previous Home Health or community nursing experience preferred

Knowledge, Skills, and Abilities

Additional Requirements

Must have and maintain a valid drivers license with an acceptable driving record.
