Location: Central City Concern - Old Town Clinic (727 W. Burnside Street, Portland, 97209)

Schedule: Variable

MINIMUM QUALIFICATIONS (REQUIRED)

Required: Current registration with the Oregon State Board of Pharmacy as a Pharmacy Intern.

PREFERRED QUALIFICATIONS

Prior experience in an outpatient pharmacy setting.

ROLE SUMMARY: The Pharmacist Trainee (P1 - P3) acting under a pharmacy intern license provides support to the Pharmacist in the processing of prescriptions and maintenance of the Pharmacy Department.

ESSENTIAL DUTIES and RESPONSIBILITIES:

Pharmacy Intern Duties

Perform all duties necessary to ensure the safe and accurate preparation of medications, including gathering patient information, entering it into the computer system, counting and pouring medications, and labeling containers.
Prepare refill requests for providers using the refill protocol.
Provide medication consultations for patients.
Perform point of care testing for blood pressure, blood glucose and INR.
Aid in anticoagulation (warfarin) management using the anticoagulation protocol under the direct supervision of a pharmacist.
Assist in department projects and presentations as assigned.
Maintain excellent customer service, including answering questions from patients, answering phones, prepare refill requests, and responding to questions from staff and patient care team.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
Attend all mandatory CCC trainings in a timely manner.
Perform other duties as assigned.

Leadership and Planning Duties

Support patient safety by assisting pharmacist with medication management protocols.
Promote adherence by consulting patients, performing drug utilization reviews, reviewing returns to stock, and communicating trends with providers.
Foster team spirit in a collaborative care setting with a diverse provider and patient population.
Actively contribute to the evaluation and continuous improvement of Pharmacy Department objectives and goals.
Facilitate the development and implantation of new pharmacy services.

Skills & Abilities:

Ability to consider the impacts and outcomes for underserved communities during decision-making process.
Ability to consider impacts of oppression, structural racism, and individual bias on client outcomes.
Methods and techniques of pharmacy operation.
Prioritize workflow, remain flexible and possess willingness to learn.
Ability to work as a team.
Knowledge of computer systems.
Principles and procedures of record keeping and reporting.
Ability to communicate effectively, orally and in writing.
