Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation, joint replacement & spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.
Position Summary:
The Patient Safety Coordinator (PSC) works under the direction of the Patient Safety Officer (PSO) to support the facility-wide patient safety program. This role will work with others within the facility to focus on process improvements that will support the reduction of medical/health care errors and other factors that contribute to unintended adverse patient outcomes. This role may participate in safety assessments, assist in coordination of the activities of the patient safety committee, educate others on system-based causes for medical errors, consult with management and staff, and communicates literature-based ideas regarding effective patient safety strategies to others within the organization. Key responsibilities of this position also include working with all services to hardwire reliable delivery of healthcare systems that incorporate best practice patient safety methodologies.
This individual must foster and maintain collaborative relationships with all employees and medical staff in implementing best practices in patient safety. Responsibilities include, but are not limited to, reviewing patient safety events in the electronic reporting monitoring system (eSRM), tracking and trending like occurrences, participate and/or lead root cause analysis meetings, and complete the root cause analysis documents. Responsible for initiating intense analysis and focused reviews for occurrences that do not reach the level of a sentinel event but require collaborative multidisciplinary involvement to prevent reoccurrence, initiating and monitoring patient safety initiatives, monitoring and ensuring compliance with The Joint Commission (TJC) National Patient Safety Goals, educating and updating staff and employees on patient safety initiatives and best practices. This individual works closely with the Patient Safety Officer to ensure all events are appropriately reviewed from a clinical perspective. Duties are under the guidance of the patient safety officer.
Schedule: Monday thru Friday. 40 hours. Exempt.
Shift: 8 hours. Days.
