*UAS nurses needed for *Initial Assessments and Reassessments. Nurses must attend a mandated 3-day orientation.
*For Full-time opportunity: Must be willing to commit to at least 15 cases weekly*
*UAS Nurse Responsibilities:*
* Perform UAS Reassessments in client's home or facility according to state guidelines.
* Document any concerns, conflicting information, other issues that surface during the evaluation process.
* Enter data into electronic UAS evaluation form.
* Assess eligibility in need of Long Term Care Services
* Ensuring compliance with regulatory requirements and healthcare industry standards
* Assisting in the coordination of interdisciplinary teams to ensure continuity and quality of patient care
* Providing education and guidance to patients and their families regarding healthcare resources and options available to them
* Reviewing patient medical records and assessing their clinical condition to determine the appropriate level of care
*UAS Nurse Qualifications:*
* UAS Certified (Uniform Assessment System for New York)
* All UAS Modules must be up to date
* Minimum 1 year UAS / Home Care Experience preferred
* New York State Registered Nursing License
* Experience with QA tool (Lanavi) preferred
* Must be willing to conduct reassessment in a nursing home setting
* Ready to work and dedicated
* Willing to train the right person
*\*UAS Nurses must have an active UAS User ID #\**
