This includes taking notes on patient conditions, answering questions from family members, and administering medicine to any patients who need it.

Responsibilities:

* Communicate with the patient, their family, and their home care nurses.
* Evaluate the home's living conditions and help to create a plan to improve them.
* Communicate with the nursing home staff to coordinate care of the patient.
* Help set up daily schedules for patients and their families.
* Delegations of medication administrators / medication passers.
* Continued education with care staff within facility.

Job Types: Part-time, Permanent

Pay: $25.00 - $32.00 per hour

Expected hours: 5 per week
