*As an After-Hours RN, Your Key Responsibilities Include:*
* *Patient Assessment:* Evaluate home health patients to determine their physical, psychosocial, and environmental needs, documenting findings accurately.
* *Ongoing Evaluation:* Conduct initial and follow-up assessments to identify changes in patient conditions and adjust care plans accordingly.
* *Communication:* Report significant health changes, safety concerns, and environmental issues to the clinical supervisor, physician, and care team.
* *Patient Safety:* Implement safety plans using information from patients, families, and community resources.
* *Plan of Care:* Ensure the Plan of Care is followed and updated as needed to maintain quality and continuity of care.
* *Problem Identification:* Recognize deviations from normal health conditions and take appropriate action.
* *Quality Care:* Deliver nursing care according to physician’s orders and established guidelines.

*Requirements:*
* Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
* 1+ year of experience as a nurse in a clinical care setting.
* Home Health/Hospice experience is preferred, but not required.
* Current State license as a Registered Nurse.
* Excellent verbal and written communication skills.
* Preferred to live in service area. This creates less driving for you and better service time for patients
* Preferred to live in service area (less driving for you and better response time for patients)
