Program Coordinator II (Social Worker)-Resilience & Early Relational Health Coordinator

Description:

This position coordinates and facilitates services delivered by the Medical-Financial Partnership Program in prenatal, perinatal, pediatric, and family medicine care settings to: 1) coordinate and supervise Medical-Financial Partnership team activities, 2) ensure continuity of services, 3) optimize service delivery, and 4) minimize gaps in care for families.

Oversees and supports Medical-Financial Partnership services across care settings and departments.

Coordinates and supervises the implementation of a life course, two generation resilience-building intervention in clinical settings. Provides direct clinical support for the Medical-Financial Partnership interdisciplinary program to promote trust-based parent-provider relationships across care settings and to promote parenting capacity of parents (prenatally and in the pediatric setting), including for those with high psychosocial and health risk. Collaborates with interdisciplinary team to promote prenatal, perinatal, and pediatric secure attachments in order to promote early relational health through services that address structural, economic, and social drivers of behavioral, mental, and overall health. Assists in the collection of relevant clinical measures for patient care and program support.

Performs professional social work service to patients and families with complex medical and related psychosocial needs in the ambulatory care setting.

Provides daily clinical direct service in collaboration with the Medical-Financial Partnership team. Provides ongoing education and supervision of social work interns/resilience coaches exercised in accordance with scope of practice guidelines. These activities include timely and accurate documentation and administrative processes related to educational roles, in coordination with other program staff and leadership. Participates in the recruitment and placement of social work interns/resilience coaches, and maintains relationships with schools of social work to facilitate intern placement processes.

Supports facilitation of Medical-Financial Partnership multi-disciplinary team conferences to discuss and evaluate clients’ progress, and to develop care coordination and care transition plans. Promotes and maintains collaborative relationships to support service connections with various health care team members, including other social workers and providers in Los Angeles Department of Health Services Clinics where the Medical-Financial Partnership Program operates. Promotes and maintains collaborative relationships with service partner organizations and agencies in the community.

Documents clinical and program service activities accurately, completely, and in a timely manner.

Essential Job Functions - Interviews individuals, their families and/or significant others to obtain a family, social, employment, medical, substance abuse, and mental health treatment history. Assessment may also include parenting and child developmental information. - Collaborates with client/parent/family to develop a tentative care plan designed to enhance client’s capacity in addressing child/family adversity and related economic, social, and health related stressors, which may include referral to another resilience bridge team member for individual counseling, case management, and/or referral to another community based organization.

Confers with representative of other departments and community agencies to resolve problems and to develop and maintain understanding and sound working relationships. - Participates as a member of administrative, educational, professional, and community committees for program planning. - Plans and conducts the medical-financial partnership program under the guidance of supervisory staff by considering the client’s goals and by determining the priorities of the programs to be dealt with. - Provides orientation to multi-disciplinary staff members to further their understanding of Medical-Financial Partnership program in mitigating impact of structural, economic, and social drivers of intergenerational health. - Provides linkage to community agencies to secure resources for clients, families, and significant others in order to achieve client’s goals.

Serve as day-to-day managers of clinical site operations.

Meet with the Medical-Financial Partnership project manager, co-directors, and other Program coordinators on at least a weekly basis. - Works with families and significant others toward gaining their acceptance of and participation in treatment recommendations. - Participates in multi-disciplinary team conferences to discuss and evaluate the nature of the client's goal(s), to evaluate the client's progress, and to develop a care plan. - May participate in research studies and special projects to develop more effective methods of identifying and evaluating the needs of patients and families.

REQUIREMENTS: - TRAINING AND EXPERIENCE: A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education including two years of clinical social work experience.

LICENSE: A valid and active license as a Licensed Clinical Social Worker issued by the California Department of Consumer Affairs, Board of Behavioral Sciences. - Master of Social Work graduates must become registered as an Associate Clinical Social Worker with the California Department of Consumer Affairs, Board of Behavioral Sciences within sixty (60) days from commencement of employment with the County of Los Angeles. Active and valid Associate Clinical Social Worker registration must be maintained continuously until successful completion of the examination process for licensure as a Licensed Clinical Social Worker. Registered Associate Clinical Social Workers must obtain licensure within six (6) years from the original date of registration with the California Department of Consumer Affairs, Board of Behavioral Sciences. Failure to obtain licensure within the aforementioned timeframes may result in termination. - A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

DESIRABLE QUALIFICATIONS:

· At least 5 years Post-degree experience working in a multidisciplinary team within a medical setting assessing psychosocial needs of patients and families

· At least 5 years Post-degree experience evaluating infant, family, early childhood mental health and developing interventions/care plans

· Able to work in a fast-paced environment with minimum supervision

· Spanish language fluency

Salary Range: Up to $108,000/yr.
