We’re seeking a Director of Care Management to lead a high-performing team focused on quality, collaboration, and compassionate care over 2 thriving acute care hospitals in the Pacific Northwest region. In this pivotal role, you’ll oversee care management and utilization review functions that ensure every patient receives safe, seamless, and cost-effective care from admission through discharge. You’ll guide a talented interdisciplinary team and partner closely with clinical and operational leaders to elevate the patient experience and optimize performance across the continuum of care. What You’ll Do: Lead care coordination, discharge planning, and utilization review processes. Drive systemwide initiatives that support clinical excellence and efficient resource use. Coach and mentor a skilled team of nurses and care management professionals. Collaborate with physicians, community partners, and payors to ensure smooth transitions of care. Use data and analytics to track trends, improve outcomes, and support organizational goals. What You’ll Bring: Bachelor’s degree in Nursing (Master’s preferred). RN with at least 2 years of acute care experience. 3+ years in care coordination or utilization review leadership at the Director level. A visionary, relationship-centered approach to leadership and change management. Commitment to advancing quality, equity, and patient-centered care.
