Evaluate the patient prior to discharge.Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
