Assistant Director of Nursing Seneca Lake Terrace is a 65 bed Assisted Living Facility in Geneva, New York. Position Title: Assistant Director of Nursing Department: Nursing Reports To: Director of Nursing Position Summary: Full Time Salary: $72,000-$75,000 Location: Geneva, NY Facility: Seneca Lake Terrace Essential Functions and Qualifications Must possess a valid LPN license, in good standing in accordance with laws of this state, from an approved TN school of nursing. Must possess, at a minimum, a high school diploma or a GED. Must comply with standards of business conduct in accordance with federal, state, and local health and regulatory standards and guidelines, as applicable. Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights. Must be able to read, write, speak, and understand English. Must maintain the highest standards in caring and servicing the needs of the residents and residents' family members and loved ones. Must adhere to all facility policies and procedures. Must adhere to Viva Senior Living’s attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook. In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather, evacuation, temporary staffing shortages, etc.). Must be able to execute and maintain accurate documentation per physician orders. Responsibilities / Accountabilities Coordinate the day-to-day care of residents in accordance with their service plan, ensuring sufficient staffing, coordinating staff schedules, and supervising RMAs and direct care staff. Maintain knowledge of individual care plans and provide support to residents accordingly, contributing to care planning by providing information and observations. Complete state-required assessment and service plan training. Coordinate services with hospice and home health, ensuring alignment with primary care providers and service plan, including wound monitoring per Viva policies. Coordinate services with durable medical equipment companies to ensure residents receive necessary equipment. Coordinate medication management with pharmacies, including managing medication orders, ensuring availability, disposal, cart management, and routine audits. Administer and document medication and treatments per physician orders, maintaining accurate records. Contribute to continuity of care by reporting changes in resident condition to responsible parties, primary care provider, and Resident Care Director. Review End of Shift Reports and Med Tech logs and respond accordingly. Respond to resident emergencies, ensuring proper response and follow-through. Document incident reports per Viva policies. Conduct community audits and training to ensure compliance with state regulations and company policies. Participate in routine activities of resident care, including daily living and basic nursing procedures. Respond to inquiries from residents, visitors, and other personnel promptly and courteously. Communicate effectively and tactfully with residents, families, visitors, and peers. Arrive at work on time as scheduled. Perform other related duties as required. Ensure residents and families receive the highest quality of service in a caring atmosphere. Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals per facility policy. Report work-related injuries and illnesses immediately to supervisor. Complete all assigned training and competency as a condition of employment. Participate in all life safety and emergency drills and trainings. Fulfill responsibilities during activation of the facility’s emergency plan. Assist with evacuation of residents during emergency situations. Maintain confidentiality of Protected Health Information. Report non-compliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Cooperate with investigations and protect residents from abuse. Report allegations of abuse, neglect, misappropriation, exploitation, or mistreatment to supervisor and/or administrator. Report retaliation or discrimination to the Human Resource Director and the Compliance Officer. Report suspicion of crime to law enforcement and the state agency. Participate in on-call rotation. Perform other duties as requested. Annual Health Requirements Provide annual verification of a negative TB skin test. Annual Physical. Physical and Sensory Requirements Must be able to move intermittently throughout the workday. Must cope with the mental and emotional stress of the position. Must see and hear or use prosthetics that allow adequate functioning. Must meet general health requirements set by facility policies, including a medical and physical exam. Must work with ill, disabled, elderly, emotionally upset, and at times agitated residents. Must be able to push, pull, move, and/or lift a minimum of 25 pounds to at least 5 feet and carry such weight for at least 50 feet.
