Patient Care Associate Part Time Evening ShiftJoin to apply for the Patient Care Associate Part Time Evening Shift role at Hope Senior Home Care and help patients achieve a high quality of life in a home‑like setting. This position is an evening shift, part‑time, and requires a compassionate, independent caregiver who can assist with daily living, personal care, and patient enrichment activities.ResponsibilitiesRecognize and respond to the needs of patients and families.Communicate changes in patients’ condition to the nursing team.Adhere to Safe Patient Handling practices and maintain a no‑manual‑lift environment.Participate in Quality Improvement programs and daily reports.Assist patients with all aspects of personal care as indicated in their care plans.Document care delivered in the electronic medical record.Report accidents, incidents, and any observed or suspected patient abuse immediately.Observe and report skin integrity.Obtain vital signs and other procedures such as temperature, pulse, respirations, blood pressure, weight, pulse oximetry, and specimen collection.Follow infection control practices.Attend ongoing training and educational classes.Maintain confidentiality and privacy of patients according to HIPAA regulations.Communicate effectively with peers, team members, patients, and families.Facilitate life enrichment activities that encourage patient engagement.Maintain cleanliness of patients’ rooms and common program areas.Prepare, serve, and clean up after meals and snacks.Participate in committee meetings.Perform other duties as assigned and trained.QualificationsCNC (Certified Nursing Assistant) certification is preferred in good standing upon hire or current enrollment in a nursing program, with completed first clinical experience.RSU PCA: CNC certification is required in good standing upon hire and ongoing.Fluent English skills required – verbal and written.Computer literacy required.Flexibility and ability to work independently and collaboratively.Must complete mandatory HSL/HRC educational requirements and programs.Remote Type: On‑siteJob DetailsSeniority level: Entry levelEmployment type: Part‑timeJob function: Health Care ProviderIndustries: Hospitals and Health CareHebrew SeniorLife is an equal opportunity employer.
