JOB SUMMARY
The Hospice RN provides skilled nursing assessment and planning for the comprehensive care of the Hospice patients and families served at the Hospice house. The Hospice RN must ensure the needs of the patient are met as identified in the patient's initial comprehensive assessment and subsequent assessments. The Hospice RN is an integral part of the Hospice interdisciplinary team providing holistic care including spiritual, emotional, psychosocial and physical care to the hospice patient, families and or significant others.
The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/ area's/ department's policies and procedures.
JOB REQUIREMENTS
RN Graduate of an accredited School of Nursing.
EXPERIENCE
Hospice care, nursing home or acute care experience preferred: 1 year nursing experience required.
SKILLS
Exhibit an interest in end of life care. Ability to work within an interdisciplinary team; excellent communication, assessment and nursing skills. Maintains confidentiality. Seeks out and attends continuing educational opportunities to maintain clinical skills.
REQUIRED CERTIFICATIONS
Current BLS, current Wyoming Licensure.
ESSENTIAL FUNCTIONS
- Performs the initial nursing assessment, comprehensive assessment and updated assessments specific to the patient and family needs. The initial assessment will be completed upon admission to the Hospice House. The comprehensive assessment will be completed within 5 calendar days of admission to include the spiritual, emotional physical and psychosocial needs of the patient. The Hospice RN must include the entire interdisciplinary team to be involved in the planning and implementation of the comprehensive assessment and subsequent updated assessments to ensure quality and consistent care as it relates to the hospice philosophy.
- The Hospice House RN will consult with the outpatient Hospice RN to update the comprehensive assessment upon admission if the patient is transferred from the outpatient hospice service and in consultation with the Interdisciplinary Team.
- Initiates the patient's Plan of Care and makes any necessary revisions.
- Informs Primary Care Provider, nursing supervisor and Interdisciplinary Team of changes in patients' condition and needs.
- Comply with Medicare Conditions of Participation.
- Provides total patient care for a team of patients with the assistance of the core hospice team and ancillary services.
- Directs and supervises non-licensed staff in the care of the Hospice resident.
- Supervises and works in coordination with the Hospice aides. Observes and delegates the Hospice aide cares and assures that the appropriate level of care is given to the resident and their families.
- Documents the resident's assessments on admission, every shift and as needed upon changes in the residents condition on a continued basis.
- Documents and updates the plan of care that include interventions that meet the physical, psychosocial, emotional and spiritual needs of the residents and families served at the Hospice House in consultation with the attending physician, Hospice Medical Director and the Hospice Interdisciplinary Team.
- Assesses and documents the goals and outcomes of the hospice resident, families and or significant others served at the Hospice House and reports changes to the Interdisciplinary Team as they occur.
- All assessments will include but are not limited to pain, dyspnea, nausea, vomiting, constipation, emotional distress, and spiritual needs.
- Attend and participate in Interdisciplinary Team meetings every two weeks to update the plan of care in consultation with the core team.
- Transcribes and carries out physician's orders, as appropriate.
- Administers and documents medications as ordered and observes for therapeutic as well as adverse effects. Provides therapeutic holistic interventions as ordered and assesses outcomes to determine effectiveness of interventions. Reports effectiveness of interventions to the Interdisciplinary Team.
- Observes, documents and reports to supervisor and physician the resident's condition and reaction to drugs, treatment and significant incidents per policy and procedure of CRH.
- Support the residents, families and significant others through the end of life process including the stages of grief and dying process.
SECONDARY FUNCTIONS
- Participates in in-service programs, discharge planning, Interdisciplinary Team meetings and staff meetings.
