Commonwealth Care Alliance (CCA) seeks a Clinical Care Manager—an RN or licensed behavioral health clinician—to deliver intensive monitoring, follow‑up, and clinical care management to a panel of members with complex medical, behavioral, and social needs, including severe and persistent mental illness. Position Summary As part of the care management dyad, the Clinical Care Manager collaborates with assigned CCA members to navigate the health system, access services, and achieve goals for independent living, recovery, and community connection. The role requires remote work when possible but also in‑person home visits as needed. Responsibilities Complex care management for members with chronic conditions. Develop, update, and implement care plans. Educate members on health and treatment options. Coordinate with providers, including DMH and DDS behavioral health teams. Engage members routinely and conduct post‑discharge assessments. Close quality gaps: medication reconciliation, follow‑up after hospitalization, blood pressure monitoring, initiation and engagement (IET). Participate in interdisciplinary care team meetings. Perform standardized assessments (MDS, Comprehensive, Functional, Crisis & Risk). Conduct home visits for clinical needs and post‑transition follow‑up. Monitor data to drive proactive outreach and update care plans. Provide resource and benefit guidance. Act as clinical resource for the interdisciplinary team. Perform additional duties requested by supervisor. Working Conditions Standard office environment. Travel required to member homes and community settings. Occasional visits to CCA or provider offices for meetings. Strong internet connection required for remote work. Member Facing YES: The job requires in‑person contact with CCA members or patients as part of the job duties. Required Education RN Clinical Care Managers: Associate degree in nursing; active RN license in good standing. BH Clinical Care Managers: Master’s degree in social work or mental health counseling; active LCSW, LMHC, or LICSW license in good standing. Desired Education Bachelor’s or Master’s degree in nursing preferred. Other related healthcare degree. Certified Care Manager (CCM). Required Licensing Active RN license in good standing in state of assignment (RN). Active LCSW, LMHC, or LICSW license in good standing (BH). MA Health Enrollment No, this is not required for the job. Required Experience At least 3 years of clinical and care management experience with complex medical, behavioral, and social needs. Desired Experience Experience in a health plan setting, including Medicare Advantage or dual‑eligible populations. Experience using a care management platform (e.g., Guiding Care a plus). Experience addressing substance use disorders. Required Knowledge, Skills & Abilities Independent clinical decision‑making aligned with licensure. Critical thinking, organization, time management, and problem‑solving skills. Ability to function independently as part of an interdisciplinary team. Strong communication—written and verbal—plus interpersonal and customer‑service skills. Proficiency in multiple systems simultaneously. Adaptability, motivation, team player mentality. High attention to detail and accuracy. Required Language English. Desired Knowledge, Skills & Abilities & Language Proficiency with electronic medical records and other electronic platforms. Online training platform usage. Virtual care platform utilization. Microsoft Office Suite proficiency. Bilingual preferred.
