About the TTC Program Trumen’s Transitional Care Program supports adults with acute or chronic conditions as they move from the hospital or skilled nursing facility back to their community setting—such as their home or assisted living. The goal is to prevent avoidable readmissions by ensuring a smooth, medically supported transition. As a TTC Provider, you will: Travel to patients' homes shortly after discharge to provide in-person follow-up care Reconcile medications and review treatment plans within your scope of practice Manage acute and chronic conditions in collaboration with Trumen-affiliated physicians and the patient’s care team Communicate with patients’ families and primary‑care providers as needed to support continuity of care Coordinate with a dedicated TTC Coordinator to organize your monthly schedule based on your availability and travel preferences.
