RNs provide direct patient care, collaborate with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various care settings. Patient care provided by RNs promotes an interdisciplinary team approach, respects patients and families’ goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements, and reflects the mission and philosophy of TRU Community Care.ResponsibilitiesComprehensively assess and anticipate all aspects of the patient’s clinical nursing needs, pain and other symptoms; collaborate with primary or hospice physician to develop individualized plan of care PRN.Incorporate patient/family preferences and goals within the plan of care.Support and respect the patient’s and family’s unique diversity, including spiritual and cultural beliefs.Complete documentation accurately and completely as per agency standard and regulatory compliance.Provide patient and family education on personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying.Contribute to the team, utilize team resources, support team decisions, and maintain positive working relationships with all team members.Work collaboratively with volunteers and provide ongoing support and guidance.Demonstrate effective verbal, written, and electronic communication skills; give and receive feedback in a timely, open manner; engage collaboratively in conflict resolution processes as needed.Be flexible and responsive in meeting and exceeding expectations of all customers, both internal and external.Participate in mandatory educational activities and required meetings.Be competent using agency information and technology systems.Perform supportive role to Hospice Care Team–Case Managers and Palliative Case Managers, answering calls and delegating tasks or making home care visits.All other duties as assigned.QualificationsCurrent RN license in the state of California.BSN or higher degree preferred.Minimum one year RN experience—hospice or home health experience preferred.Experience as telephone triage nurse preferred.Require travel to multiple work locations.Must comply with agency health screening and immunization requirements.Must possess CPR certification or be able to obtain within 60 days of hire.Must demonstrate computer competency.Physical Demands & Working ConditionsVisual, hearing and communication: corrected vision and hearing to normal range; verbal communication with all staff, volunteers, and clients.Manual dexterity and fine motor: lift up to 50 pounds and assist with patient transfers.Critical thinking skills and decisive judgment required; minimal supervision.Exposure to blood, body tissue or fluids and hazardous waste; occasional exposure to high/low temperatures, chemicals, dust, bodily injury, mechanical hazards, seasonal conditions, loud noises.BenefitsHealth, dental, vision, life coverage.Same‑day pay available.Flexible vacation time – 17 days.Paid sick time.Education reimbursement – up to $3,000 a year.Qualified Public Student Loan Forgiveness (PSLF) employer.403(b) with match.Child care available.Employee referral bonuses.Upward mobility.EEO StatementSt. Paul’s Senior Services is an Equal Opportunity Employer.Salary: $38.13 – $57.21 per hour. #J-18808-Ljbffr
