Job Summary:

MercyOne Home Care- Des Moines is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support they need to maintain their health and independence.

Key Responsibilities:

* Conduct comprehensive assessments of patients' health status and needs.
* Develop and implement individualized care plans in collaboration with interdisciplinary teams.
* Administer medications and treatments as prescribed and monitor patient responses.
* Educate patients and families on disease management, medication usage, and health maintenance.
* Monitor vital signs and manage chronic conditions, ensuring timely interventions.
* Document patient care activities accurately and in a timely manner.
* Coordinate care with physicians, therapists, and other healthcare professionals.
* Provide emotional support and counseling to patients and families.
* Ensure compliance with all healthcare regulations and standards.
* Participate in quality improvement initiatives and ongoing education.

Qualifications:

* Current Registered Nurse (RN) licensure in the state of Iowa.
* Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) acceptable.
* Previous home health or community nursing experience preferred.
* Strong clinical assessment and critical thinking skills.
* Ability to work independently and manage time effectively.
* Valid driver's license and reliable transportation.
