**Job Summary:**
MercyOne Home Care- Des Moines is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support they need to maintain their health and independence.
**Key Responsibilities:**
+ Conduct comprehensive assessments of patients' health status and needs.
+ Develop and implement individualized care plans in collaboration with interdisciplinary teams.
+ Administer medications and treatments as prescribed and monitor patient responses.
+ Educate patients and families on disease management, medication usage, and health maintenance.
+ Monitor vital signs and manage chronic conditions, ensuring timely interventions.
+ Document patient care activities accurately and in a timely manner.
+ Coordinate care with physicians, therapists, and other healthcare professionals.
+ Provide emotional support and counseling to patients and families.
+ Ensure compliance with all healthcare regulations and standards.
+ Participate in quality improvement initiatives and ongoing education.
**Qualifications:**
+ Current Registered Nurse (RN) licensure in the state of Iowa.
+ Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) acceptable.
+ Previous home health or community nursing experience preferred.
+ Strong clinical assessment and critical thinking skills.
+ Ability to work independently and manage time effectively.
+ Valid driver's license and reliable transportation.
