Elizabeth Seton Residence is a Catholic, not-for-profit skilled nursing and rehabilitation facility in Wellesley Hills, MA. The Residence offers long-term care, short-term, post-acute rehabilitation stays, and outpatient Physical, Occupational and Speech Therapy. Schedule:
Full-Time: Mon-Fri 8:30am-5:00pm (Willing to be flexible)
Duties:
Provide kind and compassionate care to our residents.
Conduct resident assessments by gathering information about physical and mental health.
Coordinates with healthcare team to create personalized care plans.
Facilitate Medicare Program to ensure accurate and timely submissions.
Provides ongoing education to the Interdisciplinary Team regarding the RAI process.
Qualifications:
Current RN licensure
Must have a minimum of two years' experience in Medicare/PPS and OBRA MDS
Must have strong organizational skills, attention to detail and the ability to meet numerous deadlines
