Trauma Informed Approach - Public Health Nursing (TIA-PHN) Program:
Our TIA-PHN Home Visiting Program acknowledges the significant influence of social determinants and adverse experiences on families. Public Health Nurses deliver compassionate, comprehensive care in a supportive environment, fostering trusting relationships that empower families and promote resilience. Emphasizing respect, collaboration, and individualized care, we address immediate health needs while focusing on the underlying factors contributing to health disparities, ultimately enhancing the overall quality of life for the individuals and families we serve.
Position Overview:
The Senior Public Health Nurse (Sr. PHN) will lead the TIA-PHN and Early Support home visiting program, ensuring that high-quality nursing services are delivered to vulnerable populations in the home setting. This role oversees program operations and directly supervises a dedicated team of Public Health Nurses, providing reflective supervision, case conferencing, and guidance to help staff navigate the emotional and clinical complexities of working with at-risk families.
Regular travel throughout San Luis Obispo County is required to facilitate home visits and program activities.
What You'll Do:
* Provide Nursing Leadership: Lead the implementation and continuous quality improvement of the TIA-PHN Home Visiting Program, ensuring nursing activities align with best practices and public health guidelines.
* Support Maternal and Child Health: Champion initiatives that enhance the health of women, infants, and children. You'll oversee home-visiting services aimed at reducing health disparities and ensuring equitable access to healthcare and resources.
* Mentor and Support Your Team: Supervise and mentor a team of Public Health Nurses, offering reflective supervision and case conferencing to support their professional growth. You will foster a collaborative and supportive work environment that prioritizes staff well-being and resilience in addressing the complexities of public health nursing.
* Coordinate Program Delivery: Manage program operations, including caseloads, referrals, and documentation, to ensure effective and timely service delivery. Your oversight will ensure that home visiting services effectively reach at-risk families and align with program goals.
* Collaborate for Community Impact: Work closely with community stakeholders and local organizations. Your collaboration will strengthen the impact of services on the health and well-being of families in San Luis Obispo County.
* Advocate for Health Equity: Advocate for the needs of underserved and vulnerable populations, ensuring they have access to essential services and resources that enhance their health outcomes. Your commitment will drive efforts to promote health equity and address systemic barriers to care.

Minimum Qualifications:
Possession of a valid California license to practice as a Registered Nurse AND a California Public Health Nurse Certificate.
Senior Public Health Nurse: Either A: One year of experience as a Public Health Nurse II;
OR B: Three years of public health related experience in a public or private health care setting. (Possession of a California credential to practice as a school nurse or an advanced degree in public health nursing or a public health related field may be substituted for one year of experience.)
