VNA & Hospice of the Southwest Region of Rutland, VT is looking to hire a part-time Weekend Licensed Nursing Assistant for our Care Plus program. For our services, we have been recognized as one of the nation's top 100 performing home health agencies by HomeCare Elite.EducationValid State of Vermont Nursing Assistant LicenseQualificationsScheduled work day is Saturday & Sunday, 12 hour days High School Diploma or GEDValid Driver's License and Current Auto InsuranceMust be at least 18 years of ageResponsibilities include but are not limited to:Under the supervision of a Registered Nurse and/or licensed Therapist, receives daily patient assignments and carries out assignments in accordance with individualized patient plan of care. Collects patient and family data and reports findings to RN and/or licensed Therapist in a timely manner.Assists in the communication of information to assigned patients.Under the supervision of an RN and/or licensed Therapist, performs interventions and procedures as directed to ensure the safe effective quality of care for patients.Reports to RN and/or licensed Therapist on a variety of aspects of patient and family information including.Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation/exercises, etc.Serves as Hospice Aide as directed. Serves as Personal Care Attendant as directed.Proactively reports changes in skin conditions to RN, Case Manager or program manager in a timely manner.
