We are committed to helping the population we serve, and our communities, achieve optimum health and enjoy the best quality of life possible.What You'll DoThe Licensed Practical Nurse Ambulatory Care provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician’s Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. May be required to make home visits.Provides direct and indirect nursing care individualized to the patient’s needsCollects and records relevant patient health data accuratelyEvaluates health data to identify patient problemsImplements and documents specified interventions Identifies deviations from expected patient outcomesAdheres to approved, research-based practice standardsPerforms and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and proceduresMaintains and documents testing reagents/supplies and equipment according to established laboratory procedures Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rightsDelivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity Assures that patients have informed consent signed for indicated proceduresMaintains current licensure, certification, and competenciesProvides proof of current BLS validation to supervisorCompletes required unit credentialing according to departmental policyProvides to supervisor evidence of completion all required annual training and competency validationPractices within the New York State-defined LPN scope of practice 100% of the timeAcquires and maintains current knowledge and skills in his/her area of practicePerforms other duties as requested and observed by supervisor or managerEducationWhat we need from youGraduate of an accredited Practical Nursing program, requiredExperiencePrevious healthcare experience, preferredLicensure/CertificatesCurrent NYS Licensure as a Practical Nurse or a valid interim permit, requiredBLS certification within 6 months of start date, requiredSkillsAnalysis & EvaluationClinical SkillsInteracting with PeopleMulti-TaskingTake InitiativePhysical RequirementsHeavy work: exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs of force frequently, and/or up to 20 lbs, of force constantly to move objectsOccasional: climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping, visual acuity (color, depth, perception)Frequent: balancing, typing/clerical/dexterity,hearing,repetitive motion, seeing/monitor/computer use, standing, talking, walking, fit testingHazardsOccasional: burns, chemical, electrical, mechanical, radiation/radiant energyFrequent: bodily fluids/bloodborne exposureWorking ConditionsOccasional: noise, dust, fumes, gases, mists, odorsTravelOccasional: travel – the job may require travel from time-to-time, but not on a regular basisTotal RewardsBenefitsBassett Healthcare Network’s commitment to our employees includes benefit programs carefully designed with the needs and values of all our employees in mind.
