What You'll Do
The Licensed Practical Nurse Ambulatory Care provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician’s Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. May be required to make home visits.
- Provides direct and indirect nursing care individualized to the patient’s needs
- Collects and records relevant patient health data accurately
- Evaluates health data to identify patient problems
- Implements and documents specified interventions
- Identifies deviations from expected patient outcomes
- Adheres to approved, research-based practice standards
- Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures
- Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures
- Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights
- Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity
- Assures that patients have informed consent signed for indicated procedures
- Maintains current licensure, certification, and competencies
- Provides proof of current BLS validation to supervisor
- Completes required unit credentialing according to departmental policy
- Provides to supervisor evidence of completion all required annual training and competency validation
- Practices within the New York State-defined LPN scope of practice 100% of the time
- Acquires and maintains current knowledge and skills in his/her area of practice
- Performs other duties as requested and observed by supervisor or manager
What we need from you
- Graduate of an accredited Practical Nursing program, required
- Previous healthcare experience, preferred
- Current NYS Licensure as a Practical Nurse or a valid interim permit, required
- BLS certification within 6 months of start date, required

- Frequent: bodily fluids/bloodborne exposure
- Occasional: noise, dust, fumes, gases, mists, odors
- Occasional: travel – the job may require travel from time-to-time, but not on a regular basis

This position is eligible for a sign on bonus up to:
USD $20,000.00
Pay Transparency
Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.
Pay Range Minimum
USD $24.25/Hr.
Pay Range Maximum
USD $36.38/Hr.
