Schedule:
After your orientation period your typical schedule will be Monday to Friday 8 AM to 5 PM
Since we are not an emergency service there is no call associated with this role.
This is not a patient facing role, you'll be primarily working in an office environment.
What you'll be doing:
As a Palliative Care Registered Nurse, you'll spend the majority of your time triaging phone calls with patients. You'll often assist them with questions about medication refills, pain management, and having goals of care discussions.
You'll need to be comfortable educating patients and their caregivers on how to care for their loved one. This might include things like disease education, advance care planning, how to administer medications, and what to expect at end-of-life.
You'll be provided with a company phone, and other equipment as applicable to perform your duties.
There are some administrative duties associated with the role including managing inboxes, fax lines, and maintaining departmental documents.
As part of the palliative care team, you will coordinate continuity of care discussions with patient's specialists and primary care providers.
At Lower Cape Fear LifeCare you will be part of an Interdisciplinary Team (IDT) consisting of physicians, nurse practitioners, medical social workers and administrative staff. You'll work together and have meetings for complex case review and internal education opportunities.
You'll have the opportunity to participate in the orientation and onboarding of future colleagues by participating in clinical orientation new hires to provide information on palliative care.
As a Palliative Care RN, you'll have the opportunity to work with our inpatient teams throughout our service area to coordinate transitional care to our outpatient staff.
Working within the palliative care team, you'll have the opportunity to cross train for other core roles within the department to assist in coverage, including at our Palliative Care Clinic in Wilmington.
