GENERAL SUMMARY:

Sunset Pediatrics’ Triage Nurses are vital in delivering quality healthcare and education to patients by providing one-on-one service over the telephone. This position requires exceptional communication skills and the ability to assess patient needs remotely. Triage Nurses work collaboratively with the healthcare team to ensure efficient and effective patient care.

This position will be full-time, 36-40 hours a week, Monday-Friday with rotating Saturdays.

Clinic hours are Monday-Wednesday 8am-6:30pm, Thursday-Friday 8am-5pm, Saturdays 8:30am-12pm


QUALIFICATIONS:

- Current RN license.

- Previous experience in clinic and/or hospital settings preferred.

- Current CPR certification.

- PEARS certification required within 6 months of hire date.

- Strong verbal communication skills, with the ability to provide a high level of patient service and empathy.

- Knowledge of medical terminology and managed care preferred.

- Ability to work in a fast-paced environment, prioritize tasks, and handle high patient volumes.

- Excellent organizational skills and attention to detail.


KEY RESPONSIBILITIES:

TELEPHONE/SCHEDULING

1. Conduct telephone triage by answering patient/parent calls in real time and gathering relevant data, including symptoms, duration, and modifying factors.

2. Utilize appropriate protocols to determine the acuity of the patient's condition and provide appropriate disposition, such as referring patients to the emergency room, scheduling clinic appointments, or providing home care instructions.

3. Demonstrate empathy, concern, and reassurance through tone of voice and chosen words during telephone interactions.

4. Consult with physicians and utilize outside resources appropriately, such as the Children's Health Alliance and AAP, as needed.

5. Document all telephone interactions in the electronic medical record (EMR) immediately following the call, ensuring relevant data is recorded and nursing notes are easily accessible to other staff members.

6. Schedule physician appointments as appropriate and as directed in the policy and procedure as well as incorporating physician directives and retaining instructions.

7. Schedule specialized test, including, but not limited to laboratory test, x-rays, CT scans, stress tests or appointments with specialists. Instruct patients in preparation for test and appointments. Assist with managed care authorizations and completion of insurance forms as necessary.


PATIENT CARE

1. Coordinate patient care, including nursing assessments, follow up procedures, patient teaching, administering medications, urinary catheterizations, etc.

2. When necessary:

a. assist physician with patient exams,

b. prepares examination rooms and equipment, set up for procedures including circumcisions.

c. escort patient to rooms, take and record vital signs, personal information in the electronic medical record (EMR).

d. Collect and process samples for POC and send out testing/labs.

e. Prepare and administer vaccines and other injections as ordered by physician.

3.. Perform daily organizational tasks as defined in the clinic. Document and bill for clinic supplies and tests as appropriate, including injections/vaccines. Verify encounter form completeness as necessary.

4. Provide direction and guidance to clinic staff in nursing care. Help maintain patient flow and workflow, work collaboratively as a team member of the staff to provide high quality and efficient service to patients and clinic. Maintains patient confidentiality.

5. Provide education to patient and families.


MISCELLANEOUS:

1. Ensure sure provider receives results of all ordered tests and files according to established process.

2. Complete and send copies or written synopsis of test results and other patient documents as directed by provider.

3. Document all patient activity including cancellations and no-shows in the EMR.

4. Maintain an adequate inventory of medical supplies and samples, monitor medication and laboratory supply outdates, and ensure exam rooms are stocked and cleaned following OSHA guidelines.

5. Ensure timely communication of test results to providers and complete necessary documentation for patient records.

6. Assist with prescription refill requests

7. Maintain a neat and orderly workstation.


OPPORTUNITY FOR HYBRID REMOTE SCHEDULE:

After successful completion of 3-6 month onboarding period, the position may become eligible for a hybrid remote schedule. This means that the Nurse may have the flexibility to work remotely for a portion of their schedule while still maintaining necessary in-person clinic hours. The specific details of the hybrid remote schedule will be determined based on the needs of the clinic and the individual's performance.


KEY RELATIONSHIPS:

- Reports directly to the Clinical Supervisor.

- Works closely with the Front Desk, Business Office, Medical Assistants, and Physicians.

- Collaborates with the healthcare team to provide optimal patient care.
