How you make a difference in this role: This classification provides guidance and support for Tennessee Department of Health Program areas of their responsibility in accordance with the state and federal guidelines and drives efforts aimed at prevention, education, and outreach to meet Department and programmatic goals and objectives. An employee in this classification in collaboration with current Section Chief and Deputy Medical Director and senior leadership, develops a strategic plan to address specific health issues within the program area they are responsible for to create an opportunity to impact their specific program area. Job Overview: The Division of Family Health and Wellness oversees the Department's initiatives related to Community Health Access and Navigation in Tennessee (CHANT) Program. This position is housed within the Children and Youth with Special Health Care Needs Section and will work closely with the Children's Special Services Director, the TennCare Bureau and Managed Care Organizations, the Evidence Based Home Visiting Project Director, the Call Center Manager, the CHANT Epidemiologist, and the Regional/Metro and County CHANT staff. This position will have primary oversight of CHANT operations by applying proactive interventions through direct engagement and navigation utilizing a multi-disciplinary team model. This position will serve as the statewide coordinator for CHANT, which includes serving as a liaison between regional and metro health department CHANT leads, other State child-serving agencies and central office programs. Key Responsibilities: Oversee overall CHANT operations through maintenance and adherence to CHANT program policies and procedures. Expand the Department's outreach, screening, assessment, and navigation of services for at risk families by addressing social determinants of health and streamlining program operations via the CHANT model in local health departments. Monitor CHANT outcomes and opportunities for continuous quality improvement through data collection, analysis, and reporting. Coordinate CHANT efforts within regional, metro, and local health departments. Provide technical assistance to counties and health department regions on specific CHANT goals, objectives, and outcomes. Host statewide quarterly calls for CHANT staff and CHANT Advisory Committee. Prepare and execute annual training plan for CHANT staff. Collaborate and partner with the Children's Special Services Director, CHANT Epidemiologist and CHANT Lead staff across the state to implement and improve guidelines, training, and data collection system for integrated care coordination. This position supervises program staff, developing Individual Performance Plans, managing the Performance Evaluation process, and approving time and labor in Edison, as well as providing performance coaching to assigned employees. Develop annual strategic plan including goals and objectives, assisting in development, and management of contracts. Minimum Qualifications: Preferred Education & Experience: Education and Experience differ for this executive service classification based on the needs of the department. This might include a bachelor's degree in a health or science field, a master's degree in business administration, Public Health Administration or Health Planning, experience with statewide leadership or directing all functions of a public health program as a whole.
