Pay Range: $40 - $44/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:


  • Shift Time & Working Days: 8:00 am to 5:00 PM, Monday to Friday.
  • Review Emails, Calendar.
  • Identify and prioritize tasks that are urgent/due.
  • Contact members and providers to check on status of services/items/healthcare needs.
  • Document all contacts within the member’s file.
  • Complete in-home assessments and document following assessment
  • Describe the performance expectations/metrics for this individual and their team:
  • Completion of contractual telephonic and face to face assessments per member/LAR request monthly.
  • Completion of follow up documentation at a minimum of every 14 days.
  • Ensure members services and DME items are in place.
  • Meet audit score goal of 95%
  • Tell me about what their first day looks like:
  • Log in at 0800, complete call with IT department from 0830-0930. Following the IT call, introduction with manager, team lead and preceptor. Walk through programs frequently used, job description etc.
  • What previous job titles or background work will in this role?
  • Private Duty Nurse, School Health Nurse, Case Manager, Care Manager, Registered Nurse, Service Coordinator.
  • Any future projected positions potentially coming up? YES If yes, note: Temp to Perm; Seeking candidate that is seeking long term employment.


Key Requirements and Technology Experience:


  • Key Skills: Pediatric, Case management & Home health.
  • Special Needs Children.
  • Insurance.
  • Education: Associate degree or higher.
  • Microsoft programs (outlook, word, excel, etc); General computer skills.
  • Registered Nurse (Texas).
