* Graduation from an accredited school of nursing.
* Current registration with the Department of Professional Regulations in the State of Illinois.
* Previous experience in caring for chronic disease patients is required.
* Minimum of 5 years of experience in a healthcare setting, preferably primary care.
* Previous experience with mobilizing community resources, and navigating through the healthcare continuum.
*Job Summary*
* Coordinates team-based care within the Accountable Care Organization (ACO), including hospital, physician, and other providers
* Monitors plan to provide exceptional and efficient patient care.
* Facilitates a shared goal model within and across settings to achieve coordinated care that is patient/ family centered.
* The position is key to bridging gaps that occur in care transition.
