The Disability Services Coordinator will be responsible for coordinating services and supports for consumers, including direct support and advocacy.

Responsibilities

  • Develop and implement individualized plans for consumers.
  • Coordinate services and supports, including transportation and medical care.
  • Advocate for consumers' rights and interests.

Requirements

  • HS diploma/GED required.
  • Valid driver's license required.
  • 1-3 years' experience working with the developmentally disabled is preferred.
