How you make a difference:
As an Admissions and Discharge Coordinator, I excel in receiving, meticulously reviewing, and efficiently organizing all referrals, documents, and communication essential for hospital admissions. I take pride in overseeing the seamless execution of admission and discharge processes, ensuring a smooth transition for patients. Additionally, my role extends to maintaining consistent communication with community referral sources, fostering collaborative relationships up until the point of admission.
Key Responsibilities:
Coordinates the admission process by liaising with receiving facilities, obtaining necessary admission information, and ensuring compliance with hospital criteria and legal requirements; collaborates with physicians to facilitate admissions.
Reviews referrals in detail, collaborating with the Clinical Director to set up admissions, maintaining communication with regional districts, and tracking patients referred for admission.
Organizes all admission paperwork, providing comprehensive reporting on admission activity to Administration; welcomes new patients, offers an initial hospital overview, and completes initial admission paperwork.
Demonstrates knowledge of risk management, infection control, precautions, fire and disaster procedures, and abuse reporting, ensuring a safe and compliant admission environment.
Manages hospital occupancy, tracks vacancies, provides reports to districts on admitted patients, monitors discharges, and reconciles bed counts for effective administrative oversight.
Qualifications & Requirements:
Education
High school diploma or equivalent required.
Bachelor’s degree in Business Administration preferred.
Experience
Three (3) years of admissions and discharge experience required, two of which must be in a psychiatric setting.
Licenses/Certifications
None required.
