To work closely with payers to understand value-based metrics for UAB Medicine patient population and to perform patient care management tasks to ensure metrics are achieved.

Attends meetings each month with key insurance plans; including medicare advantage plans, Blue Cross and Medicare ACO. Over time other plans will be added. Works closely with Managed Care Team, Primary Care and insurers to fully understand value based metrics for patient care. Proactively interacts with Primary Care medical directors and other providers to establish work flows to meet value based metrics. Proactively targets patient populations and aids in designing communication plans to create 'call to action' for patients to participate in care. Interacts with the Care Coordination Team for patients that are over utilizing emergency services and Pharmacy Team, as needed, for medication adherence challenges. Interacts with Social Work for patients with resource issues. Performs other duties as assigned.

Position Requirements:

EDUCATION AND EXPERIENCE:

Graduate of a school of Nursing program approved and accredited by the Alabama State Board of Nursing required. Bachelor's degree in nursing preferred. Work experience may NOT substitute for education requirement.

LICENSE, CERTIFICATION AND/OR REGISTRATION:

Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) from the American Heart Association (AHA) may be required.
