*Position Summary – Behavioral Health Case Manager II*

The BH Case Manager II will be responsible for providing comprehensive case management services, both telephonically and through home visits, for members with behavioral health and substance abuse needs. Specializing in areas such as Eating Disorders, Maternity, Alcohol/Drug Abuse, and Autism Spectrum Disorders, the role involves handling complex cases, applying clinical judgment, and coordinating care with stakeholders to ensure members receive effective, high-quality, and cost-efficient support. As a key part of the care team, the BH Case Manager II will help improve member health outcomes and satisfaction.

*Key Responsibilities:*
* *Case Management:* Perform case management services telephonically and/or through home visits for members with behavioral health and substance abuse or substance abuse disorder needs.
* *Clinical Expertise:* Act as a subject matter expert in specific clinical areas such as Eating Disorders, Maternity, Alcohol/Drug Abuse, Autism Spectrum Disorders, etc.
* *Complex Case Handling:* Respond to more complex cases and account-specific requests, applying clinical judgment and appropriate screening criteria to assess member needs.
* *Care Plan Development:* Conduct comprehensive assessments to identify individual member needs, developing specific care plans with clearly defined objectives and goals.
* *Monitoring and Evaluation:* Continuously monitor and evaluate the effectiveness of care plans, making modifications as necessary to ensure optimal outcomes.
* *Resource Coordination:* Coordinate with internal and external resources, including providers, to meet the identified needs of members, facilitating access to appropriate quality and cost-effective care.
* *Team Collaboration:* Serve as a resource and mentor to other BH Case Managers, participating in cross-functional teams, projects, and initiatives as needed.

*Qualifications:*
* *Education:* MA/MS in Social Work, Counseling, or a related behavioral health field, or a degree in Nursing.
* *Experience:* Minimum of 3 years of clinical experience in social work, counseling, or related fields, with extensive experience in managing complex psychiatric and substance abuse or substance abuse disorder cases.
* *Licensure:* Current active and unrestricted license in one of the following areas: RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as required by applicable state laws. For government business, LAPC and LAMFT are also acceptable, along with any other applicable state or federal requirements.
* *Preferred Skills:* Previous experience in case management, telephonic and/or in-person coaching, and managed care. Experience in health coaching and motivational interviewing techniques is highly preferred.

*Additional Requirements:*
* Demonstrated ability to work with a broad range of complex psychiatric, substance abuse, and/or medical disorders.
* Strong organizational and multitasking skills, with attention to detail.
* Ability to work independently as well as part of a collaborative team.
* Excellent communication and interpersonal skills, with a positive and proactive approach to problem-solving.

*Compensation and Benefits*

Salary
* *$32 - $34 per hour*, commensurate with experience
