Major Responsibilities:
  • Acts as a resource to employees, which includes training/orienting, providing day-to-day work direction, and giving input on performance. Assigns, monitors, and reviews progress and accuracy of work, directs efforts and provides guidance on more complex issues.
  • Assists with human resource responsibilities, which may include: interviewing and selection of new employees, staff development, resolution of employee concerns, and employee morale.
  • Manages the caseloads of staff, including evaluating patients, ensuring compliance with the established Care Plan and performing appropriate Joint Visits. Follows all State of Wisconsin and Federal Regulation pertaining to the supervision and role of Physical Therapist Assistants in Homecare.
  • Assumes primary responsibility for a patient caseload. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care.
  • Conducts initial OASIS assessment and establishes plan for treatment to obtain thorough physical, psychosocial and environment data. Establishes and follows a care plan which identifies problems, and evaluates outcomes within a certain time frame.
  • Provides ongoing assessment of patient response to treatments, medication, and teaching, and applies interventions as appropriate. Updates and revises care plan as indicated.
  • Communicates with physicians and other team members to confirm, correlate and update patient plan of treatment and provides physician with a status report on a regular basis. Communicates problems to physicians and the supervisor, and states action plans and goals to resolve the problems. Schedules case conferences with the interdisciplinary care team on a regular basis to discuss case management issues.
  • Maintains and updates accurate clinical and patient records according to agency, state and federal guidelines. Performs Home Health Aide visits to maximize the patient's functional abilities in the home.
  • Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
  • Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

Licensure, Registration, and/or Certification Required:
  • Physical Therapist license issued by the state of Wisconsin, or
  • Occupational Therapist license issued by the state of Wisconsin, and
  • Basic Life Support (BLS) for Healthcare Providerscertification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, and
  • A valid drivers license issued by the Division of Motor Vehicles, or
  • Driver license issued by the state of Illinois.

Education Required:
  • Bachelor's Degree in Physical Therapy, or
  • Bachelor's Degree in Occupational Therapy.

Experience Required:
  • Typically requires 5 years of experience in physical therapy or occupational therapy, including experience within home health care.

Knowledge, Skills & Abilities Required:
  • Proficiency in clinical and organizational skills with the ability to work independently and make sound judgments.
  • Effective interpersonal and communication skills.
  • Must possess and provide proof of a valid Driver License and a registered, functional car with adequate insurance coverage.

Physical Requirements and Working Conditions:
  • Must be able to work safely with people of various size, weight, diverse and complex medical conditions, and physical limitations. Must be able to sit, stand, walk and climb stairs, gain entry to patient's residence, travel from and to assigned visits, and attend to a variety of work related activities.
  • Must have the physical agility to move about in confined spaces, including bending, twisting, kneeling, squatting and occasionally reaching one or both over head.
  • Must be able to:
  • Lift up to 50 lbs. from floor to waist.
  • Lift up to 20 lbs. over the head.
  • Carry up to 40 lbs. a reasonable distance.
  • Must be able to:
  • Push/pull with 30 lbs. of force.
  • Perform a sliding transfer of 150 lbs. with a second person present.
  • Must be capable of the manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements and office equipment.
  • Must have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest.
  • Must be able to concentrate with frequent interruptions and remember multiple tasks and functions over a period of time.
  • Must have accurate vision to read gauges and measuring devices and to interpret written instructions.
  • Operates all equipment necessary to perform the job.
