POSITION OVERVIEW: The Clinical Pharmacist provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers. Responsibilities Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians on drug therapy, drug toxicity and side effects, and assisting in prescription of appropriate doses. Coordinates activities of assigned pharmacy technicians in preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with prescriptions; ensures compliance with laws and regulations for dispensing drugs and controlled substances and record maintenance. Mentors and trains others in training, including practical experiences. Maintains liaison with medical and nursing staff; provides timely information on pharmaceutical supplies, drug usage and compatibility, and regulations; supports Joint Commission Standards. Conducts and evaluates medication histories, assesses compliance, suggests modifications, and instructs patients in proper use of prescribed drugs; participates in Patient Care Rounds with physicians. Individualizes medication regimens using pharmacodynamic and pharmacokinetic principles; strives to improve pharmaceutical care delivery within areas of responsibility and assists others in the department. Extracts pertinent information from patient or other sources for the patient’s medical management plan. Role models two-way communication of the organization’s mission, vision, values, and strategic plan for the department. Supports Lean principles of continuous improvement and acts as a champion of change. Provides and maintains a safe environment for caregivers, patients and guests. Maintains professionalism and confidentiality; complies with laws, regulations, policies and procedures; supports corporate integrity efforts. Delivers customer service and patient care that is timely, efficient and accurate. May perform additional duties of similar complexity within the organization, as required or assigned. Qualifications Education: Required: Bachelor of Science degree in Pharmacy. Preferred: PharmD.; Completion of an ASHP accredited PGY-1 residency Licensure/Certification/Registration: Required: Current registration by the Oregon Board of Pharmacy; AHA Basic Life Support for Healthcare Provider certification (not required if working exclusively in Specialty Pharmacy). Preferred: Any certification through the Board of Pharmacy Specialties (BPS). Experience: Required: N/A; Preferred: Six (6) months or more clinical pharmacy experience. Personal Protective Equipment Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely. Physical Requirements Overall: Standing and walking (75%+), lifting 1-10 lbs, driving a vehicle for caregivers who are couriers. Frequent lifting/pushing/pulling 1-25 lbs; occasional sitting, bending, stairs; occasional 11-25 lbs lifting; talking and hearing requirements. Rarely: keyboard operation; stooping/kneeling; overhead reaching; 25-50 lbs lifting/pushing. Exposure to elemental factors (rarely 10%: heat, cold, wet/slippery areas). BBP exposure risk category and related precautions apply as appropriate.
