The Provider Relations Hospital Liaison/RN acts as part of the business development team representing Home Care & Hospice providing services to physicians, discharge planners, care managers, and other potential referral sources. The incumbent initiates communication through on-site interactions at the hospital. The Provider Relations Hospital Liaison/RN coordinates education on organization services, supports admissions, helps family and patients learn about home care and hospice options, and supports the referral process.

Assists patients and caregivers with navigating the selection of, and admission to, Northern Light Home Care & Hospice.
* Coordinates with the patient's hospital care team to facilitate the smoothest possible transition of care.
* Has strong knowledge of, and promotes all Home Care & Hospice programs, with responsibility for increasing market share, achieving targets and developing productive relationships with all.
* Documents opportunities generated from referral source relationships.
* Builds Home Care & Hospice's brand awareness.
* Works directly with patients and families to enroll in hospice services.
* Demonstrates understanding of customers' needs and positions Home Care & Hospice services to best meet their needs, demonstrating how Home Care & Hospice adds value.
* Statewide travel may be required based on position.
