About the Role

We are seeking a skilled Home Health Nursing Specialist to join our team at Andwell Health Partners. As a compassionate and dedicated RN, you will provide high-quality nursing care to patients in their own homes under the direction of a physician's plan of care.

The ideal candidate will have a current R.N. license in the State of Maine and one year of recent clinical nursing experience. ANA certification in Home Health or Community Health Nursing is encouraged.

In this full-time role, you will develop plans of care with realistic/measurable outcomes in collaboration with the patient/family, assess and identify the patient's holistic health care needs, and demonstrate competency in assessment and technical skills.
