Medical Social Worker Exceptional Family Member Program

Place of Performance: The work to be performed under this contract will be at David Grant Medical Center (DGMC), Travis AFB, CA. HCWs shall receive notification two weeks prior to the reassignment to another location within a 40-mile commuting radius of their assigned MTF. 101 Bodin Cir Ste 777, Travis AFB, CA 94535

Degree/Education (5.2.4): See Exhibit 5 of the Defense Health Agencys Medical Q-coded Series (MQS) Indefinite Delivery/Indefinite Quantity (IDIQ) contract, hereafter referred to as Exhibit 5

* Degree: Master's degree in Social Work (MSW).
* Education: Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
* Licensure: Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker/MSW
* Certification: Certifications in addition to Basic Life Support (5.2.3): None
* Experience: Must have a minimum of two (2) years full-time experience in care coordination, discharge planning or case management in medical setting.

HCW Duties (5.1 Task 1): In addition to Exhibit 5, the duties for the HCW are:

* Knowledge. Must have the knowledge and skills to effectively apply Special Needs Coordinator functions. Must be at least moderately computer literate; must be familiar with standard MS Office, Adobe PDF, general database familiarity and other good general computer/electronic office equipment functions/use.
* Mastery of theories, principles, and methodologies underlying psychosocial practice.

Knowledge and understanding of developmental growth; dynamics of human behavior, family, and other social systems; and impact of illness and disability on social functioning.

* Demonstrated abilities in program planning, implementation, and evaluation, and in conducting individual, family, group, and community assessments.
* Strong capability to work with service members and their families who are experiencing a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems.
* Working knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act (HIPAA), and accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC).
* Must demonstrate ability to communicate effectively both orally and written.

Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and potentially agitated.

* Must demonstrate ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations.
* Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
* Work Environment/Physical Requirements include prolonged walking, standing, sitting or bending, and computer use.
