**Guaranteed Hours:** No guaranteed hours

**Experience:** Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience

**License:** Must have license in hand; no pending licenses accepted

**Certifications:** CPR, BLS

**Must-Have:**
- Proficiency in HomeCare HomeBase (HCHB) EMR, with use in the last year
- Proficiency in OASIS, with use in the last year
- Reliable transportation, a valid driver's license, and current agency-required liability insurance

**Job Description:**

- Provide, coordinate, and direct home nursing care according to physician’s orders based on agency policies and procedures.
- Responsible to the Clinical Manager and supervises Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Travel up to 75 miles from home clinic; mileage reimbursed at IRS Standard rate (compensated from patient home to patient home, commute to and from first and last patient not billable).
- Handle types of cases including Start of Care, Wound Care, and Follow-up Visits.
- Expected to maintain productivity of 30 points per week, equating to 40 hours.
- Manage a caseload of 5-10 patients per day, averaging 6-8 points.
- Complete all patient documentation within 24 hours of appointment.
- Be open to traveling to a nearby clinic to pick up hours, if patient census is low.
- Attend weekly meetings and participate in on-call rotations as required.
- Dress code requires wearing scrubs.
- Weekend and on-call requirements vary; discussed during interview.
- No first-time candidates; must have recent specialty experience.
- Local candidates accepted at the same rate.
