**Job Title:** Registered Nurse

**Shift:** Day 5x8-Hour (08:00 - 17:00)

**Guaranteed Hours:** 40 hours

**Experience:** Minimum 1-2 years of medical-surgical or acute care experience; 1 year of home care experience required; orthopedic experience required

**License:** Valid state license required at the time of submission

**Certifications:** BLS, Phlebotomy

**Must-Have:**
- Experience with clinical nursing, physical therapy (PT), occupational therapy (OT) in home care
- Experience with wound vac, glucometer, pro-time machine PT/INR, IV pumps, central lines/ports
- Ability to represent self as part of a healthcare excellence team

**Job Description:**
- Perform 5-20 patient visits on average per day
- Work with a daily productivity point system (5.2 points)
- Provide a variety of services including labs/lines, start-of-care assessments, and different visit types
- Required to work every 3rd to 4th weekend as needed
- On-call duties required after hours and weekends, every 3rd to 4th weekend
- Reimbursement for mileage
- Conduct point-of-care visits, manage wound care, wound vac, IV therapy (antibiotics, TPN), and post-orthopedic/joint care
- Perform incision assessment follow-ups and post-surgical assessment/complications
- Manage post-stroke, pneumonia, trach care, teaching, and G-tube feedings
- Handle ADLs assessment and complete OASIS documentation
- Utilize EPIC charting system
- Assigned to POD managing a group of zip codes and a multidisciplinary team but may assist in other PODs if necessary
- Must comply with vaccination requirements (COVID-19 vaccine not required, but flu vaccine required with medical/religious exemptions only)
- No local candidates within a specified radius
