Clinical Pharmacist at Providence Hood River Memorial Hospital. This posting represents a 0.9 FTE, variable shift opening.

The Clinical Pharmacist has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Required Qualifications:

* PharmD in Pharmacy, Or
* Bachelor's Degree in Pharmacy.
* Oregon Pharmacist License upon hire.

Preferred Qualification:

* Acute care hospital pharmacy experience, 1 year or equivalent experience.
* Completion of ASHP-accredited PGY1 residency.
