KEY RESPONSIBILITIES:

  1. Provide intensive support to youth and their families, emphasizing adaptive, communication, and problem-solving skills.
  2. Intervene during crises, reducing recurrence chances, and linking to essential community resources.
  3. Offer person-centered, home-based behavioral health services, aligning with the Person Centered Plan.
  4. Collaborate with a multidisciplinary team, ensuring symptom reduction and goal progression.
  5. Participate actively in treatment team meetings, audits, and relevant committees.
  6. Facilitate skill-building sessions for both youth and parents, enhancing their coping mechanisms.
  7. Monitor and address psychiatric and addiction symptoms, making timely referrals when needed.
  8. Engage with non-medical community stakeholders (e.g., schools, DSS, DJJDP) for holistic care planning.
  9. Uphold the highest standards of safety and ethics, ensuring services are free from neglect or abuse.
  10. Manage documentation efficiently, ensuring service notes, timesheets, and other paperwork are timely and accurate.

QUALIFICATIONS

Education/License: Candidate must have one of the following:

  1. Masters degree in a related field and 1 year of supervised, post graduate, experience serving persons with mental health and/or substance abuse disabilities.
  2. Bachelors degree in a human service related field with 2 years of supervised, post graduate, experience serving persons with mental health and/or substance abuse disabilities.
  3. Bachelors degree in another field with four years for supervised, post-graduate experience serving persons with mental health and/or substance abuse disabilities.

*Must meet the Qualified Professional Status as defined by 10A NCAC 27G.0104 rule.

Experience/Skills: Minimum of one year's experience with target age/population. Preferred: Two or more years of direct care or case management experience

Valid Driver’s License, reliable automobile with proof of current liability insurance, acceptable driving record, current vehicle registration and inspection

Physical Effort: Requires the ability to work under stressful conditions and to work irregular hours. Requires sitting for extended periods of time, viewing computer monitors, and keyboarding.

Communication Skills: Candidates must possess excellent verbal and written communication skills in order to communicate professionally via telephone and in writing. Strong grammar and writing skills are crucial for clear and concise communication with clients, colleagues, and other healthcare professionals.

Essential Technical/Motor Skills: Candidates must achieve competency in the Electronic Health Record (EHR) program, telemedicine services, and any additional software programs utilized by the company, ensuring efficient access and updating of client information. The role requires fine dexterity, adept handling, and proficient gripping abilities to perform various tasks.

Background: The candidate must pass all federal and state background checks, drug test, TB, and their educational and work history will be confirmed incompliance with company policies.
