POSITION SUMMARY

The Team Support Coordinator (TSC) position is accountable for performing a variety of compliance management activities related to clinical documentation that adheres to state, federal, and company standards The TSC is responsible for communicating all issues that impact compliance or timely integration of the documents into the patient's electronic record as well as reporting any system or workflow issues to appropriate leadership. This position supports the Home Health Agency in managing compliance with federal regulations and quality assurance. the initial communication with referral sources regarding home health agency referrals. The position is responsible for the initial data entry, processing, distribution, and ongoing documentation management of all patient information. Also included is accountability for various operational components such as monitoring census and agency capacity. The TSC provides additional assistance to the admission transition nurses, admissions coordinators and clinical supervisors where appropriate.

ESSENTIAL JOB KNOWLEDGE
  • Promotes a culture of safety, high-reliability, patient and staff engagement and performance excellence.
  • Supports a variety of regulatory compliance and performance improvement activities related to, but not limited to: Agency paperwork, orders, scanning, and agency discharges.
  • Participate in various home health agency activities as needed.
  • Notifies clinical staff and corresponding leaders of inconsistencies in documentation data, including explaining what is missing or not meeting standards.
  • Tracks all verbal orders through work queues and reports to assure compliance is met with timely submission and completion (returned signed orders) per regulation and agency policy.
  • Facilitates communication and/or documentation management for regulatory and accreditation purposes, such as missed visit notification to providers.
  • Indexes and scans all forms and order4s in a timely manner. Timely scanning and indexing of all forms and orders.
  • Submits and tracks Face to Faces (F2F) Orders in a timely manner to ensure compliance with regulations.
  • Manages work queues related to orders needing signature.
  • Routinely escalates to appropriate leadership any inconsistencies or non-compliance related to discharges, orders, F2F or paperwork not returned timely.
  • Monitors incoming department faxes as necessary, responds to and fulfills continuation of care requests as necessary.
  • Assists with documentation of supply orders.
  • Assists with insurance documentation and other records requests.
  • Assists with data collection including ensuring accuracy of data entry and audits as assigned.
  • Collects data on various key performance metrics and runs and distributes reports as assigned.
  • Provides support for electronic medical record downtime policy and computer.
  • Other duties as assigned.

JOB REQUIREMENTS (may include preferred)

Licenses:
  • N/A

Education:
  • High school diploma or GED equivalent required.

Experience:
  • One year of experience in a medical or health office environment required.

Miscellaneous:
  • Ability to communicate timely and effectively in oral and written form and maintain confidentiality of sensitive information.
  • Ability to establish and maintain effective working relationships with fellow employees, management, field staff and the public.
  • Ability to work in a fast-paced and ever-changing environment.
  • Ability to demonstrate efficient time management skills, including the ability to handle multiple tasks and prioritize effectively.
  • Ability to be self-directed in accomplishing the coordination and decision-making responsibilities of this position and be resourceful in problem solving.
  • Knowledge of and ability to use a variety of computer programs including word processing, database management and spreadsheets.
  • Knowledge of Microsoft Word, Access, and Excel is highly desirable.
  • Proficient in medical terminology and abbreviations.
  • Knowledge of Medicare, Medical Assistance, commercial insurance coverage determinations.
  • Home health specific knowledge and experience preferred.
