Job Description for a Practice Medical Director and Primary Care Physician with a Focus on Value-Based Care
Responsibilities:
  • Oversee the clinical operations of the primary care clinic, ensuring high-quality, efficient, and cost-effective care delivery.
  • Lead and mentor a team of primary care physicians, nurse practitioners, and other healthcare professionals, fostering a collaborative and supportive work environment.
  • Develop and implement value-based care initiatives, focusing on quality, efficiency, and patient satisfaction.
  • Collaborate with healthcare teams to provide coordinated care that addresses patients' physical, mental, behavioral, and social needs.
  • Engage in continuous quality improvement efforts, analyzing data and implementing evidence-based practices to improve patient outcomes and reduce costs.
Qualifications:
  • Medical degree (MD, MBBS, or DO) from an accredited institution.
  • Valid state medical license to practice as a Primary Care Physician.
  • Board certification or eligibility in Family Medicine, Internal Medicine, or a related primary care specialty.
  • Completion of postgraduate training in primary care, such as a residency program.
  • Significant experience in primary care, including leadership roles and experience in value-based care.
Skills:
  • Strong clinical skills with the ability to diagnose and manage a wide range of health conditions.
  • Excellent leadership and communication skills to effectively interact with patients, families, and healthcare team members.
  • Ability to work collaboratively in a team-based healthcare environment.
  • Proficiency in using electronic health records and other healthcare IT systems.
  • Experience in data analysis and quality improvement initiatives.

  • May involve working with a diverse patient population, including seniors and underserved communities.
