Medical Family Therapist

We are currently looking for a Full Time Medical Family Therapist to join our Medical Family Therapy Team B team located at the Ardmore Health Clinic working 9:00AM-6:00PM - Mo Tu We Th Fr.

Summary:
Provides therapeutic services to families, couples, groups, and individuals within Chickasaw Nation's Department of Health clinics or the Chickasaw Children's Village to ensure access to quality integrated health services, promote quality healthcare, and to help improve the overall health of patients. Functions as an embedded or co-located integrated healthcare team member by working collaboratively with Department of Health providers to comprehensively care for individuals and families served. Assists in developing and implementing program design and methods to measure program effectiveness.

Responsibilities:
* Provides family, couple, individual and group therapies as needed
* Implements empirically validated measurements into therapeutic practice to assess for client progress in therapy and to measure overall therapeutic effectiveness with clients
* Assists in ongoing initiatives to build and maintain referrals for services
* Accurately documents all navigator proceedings through appropriate recordkeeping in the electronic health records system (RPMS/EHR)
* Provides immediate crisis diversion
* Coordinates services a patient receives to include, but is not limited to, shared care planning, medication reconciliation, patient support and health education, and screening and tracking outcomes
* Assists in the administration of federal grants and contracts
* Prepares monthly reports
* Provides first class customer service - courteous and helpful to all internal and external customer types

- Adheres to Chickasaw Nation dress code standards
- May provide break coverage for others as assigned
- May augment staff in other areas as needed based on staffing and needs ratio in regularly assigned unit
* Maintain knowledge of and follow all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.
