POSITION SUMMARY:

Under the direction of the Central Team Leader and Manager for Clinical services, ensures optimum patient outcomes related to medication use by preventing or resolving drug-related problems and coordinating the acquisition, distribution and control of all drug therapy to patients under the care of the pharmacist.

Position: Clinical Pharmacist

Department: Inpatient

Schedule: Full Time, 40 Hours, Days/Evenings/Nights

ESSENTIAL RESPONSIBILITIES / DUTIES:

Operations Responsibilities:

* Reviews and process medication orders, evaluating for appropriateness and compliance with hospital and accreditation standards, and state/federal regulations


* Identifies medication related safety events, reports errors, and provides recommendations for process improvement


* Assists central pharmacists, technicians, and purchasing team with supply chain management, medication shortages, and safe distribution of medication to admitted patients


* Supervises the activities of supportive personnel including student interns and technicians. Responsible for checking all medication and sterile products prior to dispensing and Pyxis refill deliveries.


* Participates in the Adverse Drug Reaction (ADR) Reporting Program, Drug Use Evaluation (DUE) Program, etc. Takes corrective action to improve performance on problem areas.


* Performs other related tasks as needed.



Education/Training Responsibilities:

* Precepts pharmacy students and precepts or co-precepts residents; collaborates on resident led Quality Improvement initiatives


* Provides timely and accurate drug information, training, and education to patients and health care providers


* Assists in the orientation and training of new pharmacists, residents, interns and technicians as assigned.



JOB REQUIREMENTS:

EDUCATION:

* PharmD degree (or Bachelor's plus 2 years hospital pharmacy experience)


* Requires a Doctorate of Pharmacy or Bachelors of Science in Pharmacy with an advanced degree


* Licensure to practice pharmacy (or eligibility within 45 days of hire) in the state of Massachusetts.


* Strong interpersonal skills towards our customers (including patients, physicians and nurses) and fellow employees of the hospital.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

* Work requires a valid license to practice pharmacy in the Commonwealth of Massachusetts


* Board of Pharmacy Specialties certification is preferred



EXPERIENCE:

* PGY1 pharmacy residency or at least three years of equivalent work experience

We are highly integrated into the front lines of patient care and into the broader leadership structure of the organization. We have been training pharmacy residents for over 30 years and has seven PGY1 residents and six PGY2 programs in Critical Care, Emergency Medicine, Infectious Diseases, Oncology, Internal Medicine, and Ambulatory Care. We also provide support to over 150 student rotations each year from Northeastern University School of Pharmacy, Massachusetts College of Pharmacy and Health Science, and other nearby schools and colleges of pharmacy.
